Mortgage Related Forms

• Mortgage Certificate Request Form (Details)

This is the form used to request a Mortgage Certificate on an entity and their property.

• Cancellation and Prescribed Note (Details)

This form may be submitted to cancel a mortgage that has prescribed.

• Lost Note Affidavit (Details)

This affidavit is used only when the note paraphed for identifications with the mortgage has been lost or destroyed and the last holder of the note can verify theat fact.

• Release of Mortgage Form (Details)

This form may be submitted to cancel a Money Judgment that has prescribed.

• Affadavit to Cancel a Money Judgment (Details)

This form may be used to cancel a Mortgage that has been paid out.

Oath of Office

This form is submitted for those wishing to be sworn into office. The form must be accompanied by a letter from the office requesting the swearing, and must instruct the Clerk of Court and Deputy Clerks of Court to swear in the individual.

These are the instructions for representing yourself when applying for a divorce.

Discharge Records

This form may be filled out and either mailed into our office or brought into our office to request a copy of a Discharge Record. The requrements and specifications for those who are allowed to request Discharge Records are the same as those for Birth Certificates.

Small Claims Court

This is the form used as an affiadavit for a small claims clerk's suit.

Passports

• Passport form DS-11 (Details)

This form is the application for attaining a passport.
